Learning outcomes:

Through a facilitated panel discussion of Lyn’s story, participants will be better able to contribute to improved patient outcomes by:

  • understanding the merits, challenges and opportunities in providing collaborative health care to people with multimorbid chronic conditions   
  • recognising the roles of different disciplines and their key principles of intervention in providing health care for multimorbid chronic health conditions
  • acquiring tips and strategies to improve interdisciplinary correspondence, consultation and referrals in collaborative health care for people with multimorbid chronic health conditions.
